The Effects of Preemptive Analgesia with Intravenous Ketorolac on Endocrine-Metabolic Response and Postoperative Pain

Abstract
The purposes of this study were to investigate the effects of perioperative continuous intravenous infusion of ketorolac on postoperative pain and endocrine-metabolic response to surgery. The study comprised 40 patients undergoing general anesthesia for total knee arthroplasty. The patients received ketorolac 30 mg(K group, n £½ 20) or saline 1 ml (C, n £½ 20) intravenously 10 min before skin incision, followed by a continuous infusion of ketorolac(2.5 mg£¯h) or saline(4 ml£¯h) for 24 h. Plasma cortisol, glucose, epinephrine and norepinephrine, creatinine, potassium were measured at 1 h before and 3, 24 h after skin incision. The postoperative pain score, meperidine consumption and the development of side effects were evaluated. The K group improved pain score and required less meperidine and had less adverse effects than C group. Plasma cortisol and catecholamine concentrations increased after operation and were not significantly different between two groups. The perioperative continuous intravenous infusion of ketorolac had no or only a slight effect on endocrine-metabolic variables despite producing improved pain relief.
